Social security card

Valid Louisiana ID or driver’s license and/or discharge papers from East Baton Rouge Parish Prison

Insurance card

Proof of address

Proof of receipt/denial of public assistance

Copy of most recent federal tax return

Proof of one month check stubs or other income

CAHS has a sliding scale fee for service to accommodate individuals and families without insurance.

CAHS accepts Medicaid, Medicare and all private insurance. You can check with your insurance carrier to determine if CAHS if a provider in your network.

Early Steps

Assistive technology, audiology, health services (family education, assistance with other Early Steps services only), medical services (evaluation only), nutrition services, occupational therapy, physical therapy, psychological services, service coordination, social work services , special instruction, speech language pathology, transportation (to and from an Early Steps service only), translation interpreter (foreign language and sign language) & vision services.

  • Eligibility Families with toddlers who have a medical condition likely resulting in a developmental delay
